New Patient Registration

register_now_blue_signPlease attend the surgery during opening hours, if possible please provide your previous doctor medical card. Should you not have this card you will be asked to complete a form (GMS1). You will also be asked to complete a Patient Health Information Form. This will provide important background health information used to assess your health care needs.

You may in relation to this form be asked to see a clinician for further assessment. You may request a health check at this time. You may express a preference for a nominated clinician. Please state this on the Patient Health Information Form.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Who can Register 

Any person entitled to NHS care, living within the Practice Catchment Area, may register with the Surgery. We do not discriminate on any other grounds
